In this role, you will be the bridge between our healthcare services and the community, identifying the needs of seniors and connecting them with appropriate resources. You'll organize community activities, conduct health screenings, and provide valuable education on wellness topics. Your work will directly contribute to improving the quality of life for elderly residents and fostering a sense of belonging within the community.

Tanggung Jawab
- Identify and engage with seniors in the Pasir Ris community to understand their needs and concerns
- Organize and facilitate community activities, workshops, and events to promote social interaction and healthy aging
- Conduct health screenings and provide basic health education to seniors
- Build and maintain relationships with community partners, volunteers, and relevant organizations
- Develop and implement outreach strategies to reach underserved elderly populations
- Document and report on community engagement activities and outcomes
- Refer seniors to appropriate healthcare services and community resources as needed
- Assist in the planning and execution of health promotion campaigns
Kualifikasi
- Diploma or Degree in Social Work, Healthcare, Gerontology, or related field
- At least 1-2 years of experience in community outreach, social services, or healthcare
- Excellent communication and interpersonal skills with ability to connect with seniors
- Strong organizational skills with ability to plan and execute community events
- Knowledge of community resources and healthcare services for seniors
- Empathetic and patient approach with ability to handle sensitive situations
- Basic computer skills for documentation and reporting
- Valid driver's license with own vehicle preferred for community visits
