MINDS is seeking a visionary Manager for Future Care Planning (FCP) to join our dynamic Community-Based Support Services team. Tanggung Jawab
- Lead and oversee the end-to-end Future Care Planning process for beneficiaries, ensuring alignment with organizational goals.
- Provide expert guidance and support to families in navigating disability-inclusive care arrangements and legal guardianship processes.
- Develop and implement strategies to enhance community-based support services, ensuring high standards of service delivery.
- Collaborate with internal multidisciplinary teams to ensure a holistic approach to beneficiary wellbeing.
- Build and maintain strong partnerships with external stakeholders, social service agencies, and government bodies.
- Monitor program metrics and provide regular reports to the Senior Manager to drive continuous improvement.
- Facilitate workshops and information sessions for caregivers to increase awareness of future planning resources.
Kualifikasi
- Degree in Social Work, Sociology, Psychology, or a related field; a Master's degree is a plus.
- Minimum 5 years of experience in social services, community development, or disability support sectors.
- Demonstrated experience in case management and stakeholder engagement in the Singapore social service landscape.
- Strong empathy, interpersonal, and communication skills to effectively support families during sensitive transitions.
- Proven ability to manage projects and coordinate multidisciplinary initiatives.
- High level of proficiency in data management and reporting tools.
