Tanggung Jawab
- Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized care plans
- Provide crisis intervention and counseling services to patients and families facing medical challenges
- Coordinate discharge planning and connect patients with community resources and support services
- Collaborate with multidisciplinary teams to ensure integrated healthcare delivery
- Advocate for patient rights and facilitate access to financial assistance programs
- Develop and implement health education programs to promote community wellness
- Maintain accurate documentation and case records in compliance with healthcare standards
Kualifikasi
- Bachelor's degree in Social Work or related field; Master's degree preferred
- Valid registration with the Singapore Association of Social Workers (SASW)
- Minimum 3 years of experience in medical social work or healthcare setting
- Certification in crisis intervention and case management
- Strong knowledge of Singapore's healthcare system and community resources
- Excellent communication skills with ability to work with diverse populations
- Experience developing and implementing patient education programs
