Are you passionate about early childhood development and maternal health? KK Women's and Children's Hospital (KKH) is seeking a dedicated Community Health Visitor to join our KidSTART team. In this pivotal role, you will play a direct part in supporting families with children aged 0-3 years, ensuring they receive the essential health, developmental, and social support required for a healthy start in life.
As a Community Health Visitor, you will bridge the gap between clinical settings and the community, conducting home visits and coordinating care plans to empower caregivers. This contract position offers a unique opportunity to make a tangible impact on the health outcomes of children in Singapore, working within a multidisciplinary team of experts at a world-class medical institution.
Tanggung Jawab
- Conduct regular home visits to monitor the health and developmental milestones of children aged 0-3 years.
- Provide support and education to parents and caregivers on nutrition, hygiene, and safe parenting practices.
- Collaborate with multidisciplinary teams including pediatricians, social workers, and allied health professionals to develop individualized care plans.
- Identify developmental delays or risk factors early and provide necessary referrals to appropriate specialist clinics.
- Maintain accurate and timely documentation of visits and progress notes within the hospital’s patient management system.
- Facilitate parenting workshops and group support sessions within the community to build local support networks.
- Monitor and track program participation to ensure families remain engaged with the KidSTART initiative.
Kualifikasi
- Diploma or Degree in Nursing, Social Work, Early Childhood Education, or a related healthcare/social sciences discipline.
- At least 2 years of relevant experience in maternal, child health, or community-based social services.
- Strong interpersonal and communication skills, with the ability to build rapport with diverse families from various socio-economic backgrounds.
- Proficiency in English and a second language is highly preferred to facilitate communication with local community members.
- Demonstrated ability to work independently in a community setting while adhering to clinical governance.
- Passionate about child development and addressing health inequalities in the early years.
- Proficiency in Microsoft Office and basic health database management.
