In this role, you will serve as a vital link between our clinical staff, patients, and the community. You will be responsible for providing psychosocial support, facilitating discharge planning, and ensuring that patients receive the resources they need to thrive during and after their hospital stay. The ideal candidate possesses the ability to build and maintain professional helping relationships, remains calm under pressure, and demonstrates high flexibility in managing complex clinical situations.

Tanggung Jawab
- Assess the psychosocial needs of patients and their families to develop comprehensive care plans.
- Facilitate smooth discharge planning by coordinating with healthcare providers, insurance companies, and community resources.
- Provide crisis intervention and emotional support to patients facing complex medical diagnoses or trauma.
- Serve as an advocate for patients to ensure their rights and preferences are respected within the hospital system.
- Maintain accurate, confidential, and timely documentation of all patient interactions and interventions.
- Provide counseling and referral services for patients requiring long-term care or financial assistance.
- Participate in multidisciplinary team meetings to ensure a holistic approach to patient recovery.
Kualifikasi
- Bachelor’s Degree in Social Work (BSW); Master’s Degree or License (LSW) is highly preferred.
- Must be a Licensed Social Worker (RA 4373) in the Philippines.
- Proven experience in a hospital or clinical medical setting is strongly preferred.
- Exceptional interpersonal skills with the ability to build rapport with diverse populations.
- High level of emotional intelligence and professional resilience.
- Proficiency in case management software and electronic medical record (EMR) systems.
- Flexibility in schedule to meet the varying needs of patients and hospital operations.
- Strong problem-solving skills and the ability to work effectively in a multidisciplinary team.
