Tanggung Jawab
- Conduct comprehensive health monitoring using Remote Patient Monitoring (RPM) tools and connected medical devices.
- Perform proactive outreach calls to check on patient status, medication adherence, and general well-being.
- Document all patient interactions, vital signs, and clinical data accurately in the electronic health record (EHR).
- Escalate urgent medical concerns to the appropriate medical team or providers promptly and effectively.
- Ensure strict adherence to HIPAA regulations and privacy standards in all patient communications.
- Educate patients on managing their chronic conditions and interpreting their health data.
- Coordinate care plans between patients, families, and external healthcare providers.
Kualifikasi
- Valid Registered Nurse (RN) license required.
- Minimum of 1-2 years of experience in nursing, telehealth, or call center healthcare roles.
- Familiarity with Remote Patient Monitoring (RPM) systems and protocols is a plus.
- BLS (Basic Life Support) and ACLS certifications are highly preferred.
- Strong verbal communication and interpersonal skills with a focus on patient empathy.
- Proficient in navigating EMR/EHR systems and Microsoft Office Suite.
- Must be a resident of Davao, Philippines.
