We are looking for a detail-oriented Remote Medical Assistant to support our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) initiatives. In this role, you will play a critical part in connecting patients with the care they need, managing their daily health data, and ensuring seamless communication between the patient and the clinical team.
You will be responsible for coordinating remote patient visits, uploading vital signs data (blood pressure, glucose, weight) to the RPM platform, and documenting care plans accurately. This position requires a proactive approach to patient engagement and the ability to troubleshoot technical issues related to remote monitoring devices. Tanggung Jawab
- Coordinate and schedule remote patient visits, telehealth appointments, and follow-up calls.
- Assist in the collection and uploading of Remote Patient Monitoring (RPM) data, including blood pressure, glucose, and weight readings.
- Document patient interactions and care plans accurately within Electronic Health Records (EHR) systems.
- Provide patient education on chronic disease management and device usage.
- Screen patient inquiries and escalate urgent medical issues to the supervising nurse or physician.
- Ensure compliance with CCM and RPM regulatory guidelines and documentation standards.
- Collaborate with the care team to identify gaps in care and improve patient engagement.
Kualifikasi
- Previous experience as a Medical Assistant, Medical Receptionist, or in a Customer Support role within a healthcare setting.
- Familiarity with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) principles is highly preferred.
- Strong proficiency in English (verbal and written) for clear patient communication.
- Basic computer skills and ability to navigate medical software and EMR systems.
- Excellent organizational and multitasking abilities with a focus on accuracy.
- Empathetic communication style and a commitment to patient privacy (HIPAA compliance).
- Must be willing to work in the Philippines (Metro Manila) and have a stable internet connection.
