Are you a detail-oriented Pharmacist looking to transition into a dynamic remote role? Gaba Telepsychiatry PC is seeking a dedicated Remote Pharmacist Coordinator to join our expanding telehealth team. In this pivotal role, you will bridge the gap between clinical psychiatric care and medication management, ensuring our patients receive seamless pharmaceutical support from the comfort of their homes.
As a key member of our healthcare delivery team, you will collaborate closely with psychiatrists and clinicians to develop, monitor, and optimize personalized medication management plans. Tanggung Jawab
- Develop and oversee comprehensive medication management plans tailored to individual patient needs in a telepsychiatry setting.
- Coordinate closely with the clinical team to ensure accurate medication reconciliation and safety monitoring.
- Provide expert pharmaceutical guidance to patients regarding medication adherence, potential side effects, and drug interactions.
- Manage electronic health record (EHR) entries to ensure clinical documentation is accurate, compliant, and up-to-date.
- Act as a primary point of contact for patient inquiries regarding medication refills, dosage adjustments, and pharmacy communication.
- Monitor and analyze patient data to identify opportunities for therapeutic optimization and improved mental health outcomes.
- Participate in virtual case review meetings to discuss complex patient needs with interdisciplinary teams.
Kualifikasi
- Active Pharmacist license and valid credentials to practice.
- Minimum of 2-3 years of experience in a clinical or community pharmacy setting.
- Demonstrated experience in medication therapy management (MTM) is highly preferred.
- Strong technical proficiency with telehealth platforms, EHR systems, and digital collaboration tools.
- Exceptional verbal and written communication skills with the ability to explain complex medical information to patients.
- Highly organized, self-motivated, and capable of working effectively in a 100% remote work environment.
- Strong understanding of psychiatric medication classes and their clinical applications.
- Must possess a stable internet connection and a dedicated, HIPAA-compliant home office space.
