- Assess, plan, implement, and evaluate comprehensive care plans for heart failure patients
- Coordinate patient care across healthcare settings, including hospital, outpatient, and community settings
- Provide education and support to patients and families regarding heart failure management
- Collaborate with multidisciplinary team members to ensure coordinated, patient-centered care
- Monitor patient outcomes and adjust care plans as needed to optimize health status
- Document patient interactions and care plans accurately and timely
- Advocate for patients' needs and facilitate access to appropriate resources and services
- Participate in quality improvement initiatives related to heart failure management
Kualifikasi
- Bachelor's degree in Nursing or related healthcare field
- Minimum of 3-5 years of clinical experience in cardiology or heart failure management
- Certification in Case Management or related field preferred
- Strong knowledge of heart failure pathophysiology, treatment modalities, and evidence-based practices
- Excellent communication and interpersonal skills with ability to interact effectively with patients, families, and healthcare team
- Strong organizational and time management skills with ability to prioritize and manage multiple cases
- Proficiency in electronic health records and care management software
- Demonstrated ability to work independently and as part of a multidisciplinary team
