Tanggung Jawab
- Manage a dedicated caseload of patients, providing ongoing monitoring, assessment, and clinical support.
- Develop and implement personalized recovery plans in collaboration with psychiatrists, psychologists, and social workers.
- Facilitate psychoeducation sessions for patients and their caregivers to improve understanding of mental health conditions.
- Coordinate transitions of care between inpatient, outpatient, and community-based services.
- Maintain accurate and timely clinical documentation in accordance with hospital standards and protocols.
- Advocate for patients' needs within the healthcare system to ensure equitable access to support services.
- Participate in multidisciplinary team meetings to discuss complex cases and refine treatment strategies.
Kualifikasi
- Degree in Nursing, Psychology, Social Work, or a relevant healthcare discipline.
- At least 3-5 years of experience in mental health case management or a related clinical environment.
- Strong interpersonal skills with the ability to build rapport with patients experiencing acute mental health crises.
- Excellent verbal and written communication skills in English.
- Demonstrated ability to work effectively within a multidisciplinary medical team.
- Proficiency in clinical management software and electronic health records.
- Resilience and the ability to manage emotional stressors in a high-pressure clinical setting.
- Prior experience with early intervention programs or psychosis-specific care is highly advantageous.
