Tanggung Jawab

  • Conduct home visits and community outreach activities to support families with children aged 0-3 years
  • Provide health education and parenting guidance to caregivers and families
  • Monitor and track child development milestones using standardized assessment tools
  • Collaborate with pediatricians, nurses, and social workers to develop and implement individualized care plans
  • Identify early signs of developmental delays or health concerns and coordinate appropriate referrals
  • Maintain accurate case documentation and communicate effectively with healthcare teams
  • Participate in team meetings, training sessions, and program development initiatives
  • Build strong relationships with community partners and resources to support family needs

Kualifikasi

  • Diploma or Degree in Nursing, Health Sciences, Social Work, or related field
  • Minimum 3-5 years of experience in community health, pediatric nursing, or child health services
  • Certification or experience as a Health Visitor, Community Health Nurse, or similar role is advantageous
  • Strong interpersonal and communication skills with the ability to engage sensitively with diverse families
  • Proficiency in Microsoft Office applications and electronic health record systems
  • Self-motivated, organized, and able to work independently in community settings
  • Valid Singapore nursing registration (if applicable) or relevant professional certification
  • Fluency in English; additional languages such as Mandarin, Malay, or Tamil is an advantage
