Tanggung Jawab
- Conduct comprehensive psychosocial assessments of residents and their families to identify needs and strengths.
- Develop, implement, and review individualized care plans, collaborating with medical, nursing, and allied health professionals.
- Provide counseling, emotional support, and crisis intervention to residents and families coping with illness, grief, and end-of-life issues.
- Facilitate effective discharge planning, ensuring seamless transitions of care and access to appropriate community resources.
- Liaise and collaborate with external agencies, community partners, and government bodies to advocate for residents' welfare and secure necessary support services.
- Educate families on available support services, financial assistance schemes, and caregiving resources.
- Participate actively in multidisciplinary team meetings, contributing a social work perspective to resident care discussions.
- Maintain accurate and up-to-date documentation of all social work interventions and progress.
Kualifikasi
- Bachelor's or Master's degree in Social Work from a recognized institution.
- Minimum of 3-5 years of relevant experience as a Medical Social Worker, preferably within a hospital, nursing home, or eldercare setting.
- Registered with the Singapore Association of Social Workers (SASW) or eligible for registration.
- Excellent interpersonal, communication (both written and verbal), and active listening skills.
- Strong empathy, cultural sensitivity, and a genuine passion for working with the elderly.
- Proven ability to work independently and collaboratively as part of a multidisciplinary team.
- Demonstrated problem-solving, critical thinking, and crisis management abilities.
- Proficiency in English; ability to communicate in other local languages (e.g., Mandarin, Malay, Tamil) would be an advantage to engage with diverse residents.
