As a Pharmacist at SGH, you will be at the forefront of patient care, working within a multidisciplinary team to manage acute and chronic diseases. Tanggung Jawab
- Provide expert clinical pharmacy services within multidisciplinary teams to optimize patient outcomes.
- Review and validate medication orders to ensure therapeutic appropriateness, safety, and dosage accuracy.
- Conduct patient medication counseling and education to improve adherence and health literacy.
- Collaborate with physicians and nursing staff on medication reconciliation and therapeutic monitoring.
- Manage and oversee the dispensing of medications in compliance with legal and hospital regulations.
- Participate in clinical audits, research projects, and quality improvement initiatives.
- Lead and mentor junior pharmacists, pharmacy technicians, and interns (Senior roles).
- Contribute to the development of clinical protocols and drug formulary management.
Kualifikasi
- Bachelor of Science in Pharmacy from a university recognized by the Singapore Pharmacy Council (SPC).
- Fully registered Pharmacist with the Singapore Pharmacy Council (SPC) and a valid practicing certificate.
- Minimum 3-5 years of clinical experience in a hospital setting for Senior Pharmacist consideration.
- Strong clinical knowledge in therapeutic management of acute and chronic conditions.
- Excellent interpersonal and communication skills to interact effectively with patients and healthcare teams.
- Proven ability to work in a fast-paced, high-pressure environment.
- Commitment to evidence-based practice and lifelong learning.
