Join Changi General Hospital's pioneering Mobile Inpatient Care @ Home (CGH@Home) program and redefine hospital care delivery. As a Senior/Staff Nurse, you'll provide acute-level nursing services directly to patients' homes, bridging the gap between hospital and community care. This innovative role requires you to collaborate seamlessly with an interdisciplinary team to deliver personalized, evidence-based care while maintaining clinical excellence in home environments.
You'll manage complex cases including post-surgical recovery, chronic disease management, and acute interventions, leveraging telehealth technologies for remote monitoring. Tanggung Jawab
- Deliver acute hospital-level nursing care in home settings including wound management, medication administration, and IV therapy
- Collaborate with physicians, therapists, and social workers to develop comprehensive care plans
- Conduct regular patient assessments using telehealth platforms and mobile diagnostic tools
- Provide patient/family education on disease management, medication protocols, and home safety
- Respond to emergencies and provide immediate intervention while coordinating with on-call specialists
- Document care progress using electronic health records and maintain accurate clinical documentation
- Participate in quality improvement initiatives for home-based care delivery models
Kualifikasi
- Singapore Nursing Board registration with current practicing license
- Bachelor's degree in Nursing (Master's preferred for senior roles)
- Minimum 5 years clinical nursing experience with 2+ years in acute care or home health
- Certification in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS)
- Experience with telehealth technologies and mobile clinical equipment
- Strong critical thinking skills for independent decision-making in home environments
- Excellent communication abilities for multidisciplinary collaboration and patient education
