As a Social Work Assistant for the KidSTART@KKH KIDS 0-3 programme, you will play a pivotal role in supporting the early developmental needs of children from low-income families.
In this position, you will work closely with a multidisciplinary team to coordinate essential services, monitor client progress, and ensure that families are well-supported during the critical early years of a child's life. This role is perfect for a compassionate individual dedicated to social advocacy and community healthcare in Singapore.
Tanggung Jawab
- Coordinate and facilitate the delivery of support services for eligible children and their families under the KidSTART programme.
- Conduct regular outreach and maintain consistent communication with families to monitor developmental milestones and well-being.
- Collaborate effectively with medical professionals, social workers, and community partners to provide holistic case management.
- Maintain accurate and detailed case documentation and reporting in accordance with hospital standards.
- Identify barriers to service access and assist families in navigating community resources and support networks.
- Participate in multidisciplinary team meetings to review case progress and refine intervention strategies.
- Assist in the organization and implementation of community engagement activities and family support initiatives.
Kualifikasi
- Diploma in Social Work, Psychology, Sociology, or a relevant healthcare-related discipline.
- Prior experience in social service, early childhood education, or community outreach is highly advantageous.
- Strong empathy and a genuine passion for working with children and families from diverse backgrounds.
- Excellent communication and interpersonal skills to build rapport with clients and stakeholders.
- Proficiency in Microsoft Office applications and database management software.
- Ability to work independently and as part of a team in a fast-paced healthcare environment.
- Strong problem-solving skills with a proactive and organized approach to case management.
