St Joseph's Home is a dedicated aged care facility committed to providing compassionate and holistic care for elderly residents. We are currently seeking a passionate and qualified Social Worker to join our team in Jurong West. As a Social Worker at St Joseph's Home, you will have the opportunity to make a meaningful difference in the lives of our residents by delivering high-quality social work services in accordance with the Social Work Competency Framework. In this role, you will conduct comprehensive biopsychosocial assessments, develop and implement individualized care plans, and provide emotional and practical support to residents and their families. You will work collaboratively with a multidisciplinary team of healthcare professionals, including nurses, doctors, therapists, and fellow social workers, to ensure integrated and resident-centered care. Your responsibilities will also include advocating for residents' access to community resources, facilitating group programs, and contributing to quality improvement initiatives.

Tanggung Jawab
- Conduct comprehensive biopsychosocial assessments for new and existing residents.
- Develop, implement, and review individualized care plans in collaboration with residents, families, and the care team.
- Provide counseling and psychosocial support to residents and their families to address emotional, social, and practical needs.
- Advocate for residents' rights and facilitate access to community resources and financial assistance schemes.
- Collaborate with medical, nursing, and allied health professionals to ensure integrated care.
- Organize and facilitate group activities and support programs to enhance social engagement and well-being.
- Maintain accurate and timely documentation in accordance with professional standards and organizational policies.
- Participate in quality improvement initiatives and contribute to the development of best practices in gerontological social work.
Kualifikasi
- Recognized degree in Social Work, preferably with a focus on gerontology or healthcare.
- Accredited or eligible for accreditation with the Singapore Association of Social Workers (SASW).
- Minimum 2 years of experience in social work, preferably in a geriatric or healthcare setting.
- Strong understanding of the Social Work Competency Framework and aged care policies in Singapore.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively in a multidisciplinary team.
- Proficiency in documentation and case management software.
- Empathy, patience, and a genuine passion for working with the elderly.
