In this role, you will play a pivotal part in providing holistic support, advocacy, and case management for residents. You will work within a multidisciplinary environment, ensuring that each resident's care plan is tailored to promote their well-being, rehabilitation, and social integration. Tanggung Jawab
- Develop and implement comprehensive care plans tailored to the individual needs of residents.
- Provide ongoing counseling and emotional support to residents and their families.
- Conduct regular assessments and reviews to monitor progress and adjust support strategies accordingly.
- Facilitate multidisciplinary team meetings to ensure coordinated and effective care delivery.
- Maintain accurate and up-to-date case documentation in compliance with organizational and regulatory standards.
- Advocate for resident needs and facilitate access to community resources and external support services.
- Assist in the management of crisis situations and provide immediate intervention when necessary.
- Promote resident empowerment and encourage participation in daily living activities and therapeutic programs.
Kualifikasi
- Bachelor’s Degree in Social Work or a related field (accredited programs preferred).
- Registered Social Worker (RSW) certification or eligibility for registration.
- Minimum 1-2 years of experience in social work, preferably in a residential, healthcare, or institutional setting.
- Strong interpersonal and communication skills with the ability to empathize with diverse populations.
- Proficiency in case management software and Microsoft Office suite.
- High level of resilience and the ability to work effectively under pressure in a fast-paced environment.
- Proven ability to work collaboratively within a multidisciplinary team.
- A deep commitment to professional ethics and maintaining resident confidentiality.
