As a Senior Medical Social Worker, you will play a pivotal role in providing advanced psychosocial support to patients and their families navigating complex health challenges associated with acute and chronic illnesses, trauma, and aging. You will manage a diverse caseload, offering expert counselling, crisis intervention, and comprehensive discharge planning to ensure holistic, patient-centred care. This role goes beyond direct practice; you will be a mentor to junior staff, contribute to quality improvement initiatives, and champion the biopsychosocial model within the multi-disciplinary team.

Tanggung Jawab
- Conduct comprehensive psychosocial assessments to evaluate patient needs, support systems, and coping mechanisms, formulating individualized care plans in collaboration with the multi-disciplinary team.
- Provide expert individual, family, and group counselling addressing emotional, social, and financial stressors related to hospitalization, chronic illness, and life transitions.
- Lead complex discharge planning initiatives, coordinating post-discharge care, community resources, and financial aid applications (e.g., Medifund, Community Health Assist Scheme).
- Serve as a key advocate for patients and families, ensuring their psychosocial needs and rights are respected and addressed within the healthcare system.
- Mentor and supervise junior Medical Social Workers and social work interns, offering clinical guidance, case consultations, and professional development support.
- Contribute to departmental quality improvement projects, clinical audits, and research to promote evidence-based social work practice and enhance patient outcomes.
- Develop and deliver psychoeducational programmes and resources for patients, families, and the community to promote mental health and well-being.
Kualifikasi
- Bachelor's or Master's degree in Social Work from an accredited institution recognized by the Singapore Association of Social Workers (SASW).
- Minimum 3 to 5 years of relevant post-graduate clinical social work experience, preferably in a hospital, hospice, or medical social work setting.
- Accredited or eligible for accreditation as a Registered Social Worker with the SASW.
- Demonstrated skills in psychosocial assessment, crisis intervention, case management, and complex discharge planning within a multi-disciplinary framework.
- Deep understanding of the Singapore healthcare landscape, including community care resources, financial schemes, and relevant legislation (e.g., Mental Capacity Act, VWOs).
- Excellent interpersonal, written, and verbal communication skills in English. Proficiency in Mandarin, Malay, Tamil, or other local dialects is highly valued.
- Strong organizational, problem-solving, and leadership abilities with a passion for mentoring and service excellence.
