In this role, you will provide high-quality, personalized nursing care to patients in the comfort of their own homes. Tanggung Jawab
- Develop and implement individualized nursing care plans tailored to the specific needs of patients in a home care setting.
- Perform clinical assessments, wound care, medication administration, and vital sign monitoring as required.
- Coordinate with multidisciplinary teams, including physicians, physiotherapists, and family caregivers, to ensure holistic patient outcomes.
- Provide patient and caregiver education on health management, safety practices, and treatment compliance.
- Maintain accurate, timely, and confidential digital medical records and nursing reports.
- Respond promptly to patient health concerns and escalate medical issues to attending physicians as necessary.
- Ensure a clean, safe, and supportive environment for patients during all home visits.
Kualifikasi
- Diploma or Degree in Nursing from an accredited institution.
- Must be registered with the Singapore Nursing Board (SNB).
- Valid Basic Cardiac Life Support (BCLS) and AED certification.
- Minimum 1-2 years of clinical experience; home care or community nursing experience is highly advantageous.
- Strong interpersonal skills with the ability to communicate empathetically with patients of diverse backgrounds.
- Possess a proactive attitude, strong critical thinking skills, and the ability to work independently.
- Proficiency in basic computer skills for electronic medical documentation.
- Willingness to travel to patient locations within the Central Region.
