This is a full-time position offering a competitive salary range of $3,800 – $5,000 per month, reflecting your valuable skills and dedication.
Tanggung Jawab
- Conduct comprehensive nursing assessments and develop individualized care plans for patients in a home care setting.
- Administer medications, perform wound care, and manage medical equipment as prescribed by physicians.
- Monitor and evaluate patient conditions, documenting observations and reporting changes to the healthcare team and family members.
- Provide specialized care for geriatric patients, addressing age-related health issues and promoting independent living.
- Offer empathetic and holistic palliative care, focusing on symptom management, comfort, and emotional support for patients and their families.
- Educate patients and caregivers on disease management, medication adherence, health promotion, and prevention strategies.
- Ensure strict adherence to infection control protocols and maintain a safe and therapeutic home environment for patients.
- Collaborate effectively with interdisciplinary teams, including doctors, therapists, and social workers, to ensure coordinated patient care.
Kualifikasi
- Diploma or Bachelor's Degree in Nursing from a recognized institution.
- Valid registration with the Singapore Nursing Board (SNB) and a valid Practicing Certificate.
- Minimum of 2-3 years of clinical experience as a Registered Nurse, preferably with experience in home care, community nursing, gerontology, or palliative care settings.
- Strong clinical assessment skills and proficiency in administering various nursing procedures.
- Excellent communication, interpersonal, and empathy skills to interact effectively with patients, families, and colleagues.
- Ability to work independently and manage a caseload of patients effectively and efficiently.
- Comfortable with travel to various home locations within the Central Region of Singapore.
- Possess a valid Basic Cardiac Life Support (BCLS) certification.
