Tanggung Jawab
- Review and verify physician orders to ensure accuracy and safety prior to dispensing medications.
- Provide comprehensive patient counseling on medication usage, side effects, and interactions.
- Monitor patient medication therapy for efficacy, potential adverse reactions, and drug interactions.
- Collaborate with the multidisciplinary healthcare team to develop individualized treatment plans.
- Ensure strict compliance with hospital policies, regulatory standards, and legal pharmaceutical regulations.
- Manage pharmacy inventory, including ordering, receiving, and storage of pharmaceutical supplies.
- Participate in quality improvement initiatives and clinical pharmacy services.
Kualifikasi
- Bachelor of Science in Pharmacy (BSP) or Master of Pharmacy (MPharm) degree.
- Valid Licensed Pharmacist registration with the Philippine Regulatory Commission (or local equivalent).
- Minimum of 2-3 years of clinical pharmacy experience in a hospital or healthcare setting.
- Strong knowledge of pharmacology, drug interactions, and clinical therapeutic guidelines.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work effectively under pressure and manage multiple priorities in a fast-paced environment.
- Proficiency in pharmacy management software and electronic health records (EHR).
