Join Omega Healthcare Management Services Inc. as a Utilization Review Staff Nurse specializing in Transitional Care Management. This pivotal onsite role in Pasig City bridges the gap between hospital discharge and community care, ensuring patients receive seamless support during critical transitions. You'll continuously monitor patient in-baskets to address prescription refills, follow-up visit coordination, non-urgent medical inquiries, and test result reviews. As a key care coordinator, you'll collaborate with multidisciplinary teams to develop personalized transition plans, educate patients/families on post-discharge protocols, and maintain accurate documentation in electronic health records. Tanggung Jawab
- Coordinate transitional care plans for patients moving from acute care to community settings
- Continuously review and respond to patient in-basket communications regarding prescriptions, follow-ups, and test results
- Collaborate with physicians, specialists, and care teams to ensure continuity of care
- Provide patient/family education on care plans, medication management, and post-discharge instructions
- Utilize electronic health records to document care coordination activities and track patient progress
- Identify gaps in care and implement interventions to prevent readmissions
- Monitor patient compliance with treatment plans and adjust care protocols as needed
Kualifikasi
- Valid Philippine nursing license (RN) with active PRC registration
- Bachelor's degree in Nursing (BSN) preferred
- Minimum 2 years clinical experience in acute care or transitional care settings
- Certification in case management or utilization review preferred
- Strong proficiency in electronic health record systems and care coordination tools
- Excellent communication skills for patient/family education and multidisciplinary collaboration
- Ability to manage multiple priorities in fast-paced healthcare environments
- Detail-oriented documentation and problem-solving skills
