Tanggung Jawab
- Manage chronic care patient registries and conduct monthly outreach calls to assess patient health status.
- Maintain precise electronic health record (EHR) documentation to ensure all clinical interactions are billable and compliant.
- Collaborate with multidisciplinary healthcare teams to update and implement comprehensive care plans.
- Educate patients on medication adherence, lifestyle management, and disease prevention strategies.
- Perform triage for clinical inquiries and escalate high-risk cases to attending physicians when necessary.
- Audit clinical records to ensure adherence to insurance billing codes and regulatory requirements.
- Participate in regular quality assurance meetings to refine clinical operations workflows.
Kualifikasi
- Active Registered Nurse (RN) license in good standing.
- Minimum of 2 years of clinical experience, preferably in primary care, chronic disease management, or telehealth.
- Proficiency in Electronic Health Records (EHR) systems and standard office software (Google Suite, MS Office).
- Exceptional verbal and written communication skills with a professional bedside manner.
- Strong analytical skills with a focus on compliance and medical documentation accuracy.
- Ability to work independently in a remote/virtual environment while meeting productivity targets.
- Stable internet connection and a secure, HIPAA-compliant home office workspace.
