We are currently expanding our team and seeking a detail-oriented and compassionate Care Planning & Quality Assurance Officer to oversee care plan development and ensure top-tier quality in our service delivery.
As a Care Planning & Quality Assurance Officer, you will be responsible for creating comprehensive care plans from scratch, utilizing your assessment skills to identify risks, needs, and goals for each client. You will conduct thorough risk assessments and collaborate with interdisciplinary teams to implement tailored interventions. Additionally, you will monitor the effectiveness of care plans and adjust them as necessary to achieve optimal outcomes.
In the quality assurance aspect of your role, you will design and implement quality improvement initiatives, audit processes, and maintain compliance with industry regulations and company policies. You will analyze performance data, prepare reports, and provide recommendations to enhance service quality. Your expertise will ensure that our services consistently exceed expectations.
We are looking for a candidate with strong analytical abilities, excellent communication skills, and a proven track record in care planning or quality assurance. A bachelor's degree in healthcare, social work, or a related field is required, along with relevant experience. Certifications in quality management (e.g., CPHQ, Six Sigma) are highly valued.

- Develop and implement individualized care plans based on thorough assessments of client needs
- Conduct comprehensive risk assessments to identify potential hazards and develop mitigation strategies
- Monitor the quality of care services to ensure compliance with internal standards and regulatory requirements
- Collaborate with healthcare professionals and support teams to optimize care delivery
- Maintain accurate documentation and records of care plans, assessments, and quality reviews
- Provide training and guidance to staff on care planning processes and quality assurance protocols
- Analyze data and generate reports to track performance metrics and identify areas for improvement
Kualifikasi
- Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field
- Minimum of 2 years experience in care planning, risk assessment, or quality assurance
- Strong analytical and problem-solving skills with attention to detail
- Excellent verbal and written communication skills
- Ability to work independently and as part of a team
- Knowledge of relevant healthcare regulations and quality standards
- Proficiency in Microsoft Office Suite and data management tools
- Relevant certifications (e.g., CPHQ, QM) are an advantage
