- Evaluate and optimize patient drug therapies through medication therapy management (MTM) and comprehensive medication reviews.
- Collaborate directly with physicians, nurses, and other healthcare professionals to develop individualized care plans.
- Monitor patient outcomes, including adverse drug reactions and therapeutic responses, adjusting treatments as necessary.
- Provide evidence-based drug information and education to patients, families, and the medical staff.
- Manage medication reconciliation processes during patient admissions, transfers, and discharges.
- Participate actively in clinical rounds, contributing pharmaceutical expertise to patient care discussions.
- Ensure compliance with hospital policies, regulatory standards, and safety protocols in medication management.
- Conduct drug utilization reviews and implement cost-effective therapeutic strategies.
Kualifikasi
- Doctor of Pharmacy (Pharm.D.) degree or a Bachelor of Science in Pharmacy from an accredited institution.
- Valid and current Registered Pharmacist (RPh) license in the Philippines.
- Minimum of 1-3 years of experience in a clinical or hospital pharmacy setting.
- Strong working knowledge of pharmacotherapy, disease state management, and clinical guidelines.
- Excellent interpersonal, verbal, and written communication skills.
- Ability to work effectively and collaboratively within a multidisciplinary healthcare team.
- Detail-oriented with strong analytical, critical thinking, and problem-solving abilities.
- Proficiency in hospital pharmacy information systems and medication safety technologies.
