AWWA is looking for a dedicated Coach to join our dynamic team at the Day Activity Centre (DAC) in Pasir Ris. In this meaningful role, you will deliver holistic care that nurtures both the physical and psychosocial wellâbeing of our clients. By planning and executing a diverse range of therapeutic, recreational and social activities, you will help each individual achieve their personal goals and enjoy a higher quality of life.
You will work closely with a multidisciplinary team of healthcare professionals, families and caregivers to assess client needs, develop individualized activity plans, and monitor progress. Your dayâtoâday responsibilities include maintaining accurate records, ensuring a safe and supportive environment, and fostering a vibrant community atmosphere. Tanggung Jawab
- Plan, develop and implement daily activity programs tailored to client abilities and interests.
- Provide direct care and support, assisting with mobility, personal hygiene and medication reminders as needed.
- Conduct assessments and maintain detailed progress records for each client.
- Collaborate with therapists, nurses and families to coordinate holistic care plans.
- Ensure a safe, clean and inclusive environment compliant with health and safety regulations.
- Monitor client wellâbeing and respond promptly to any changes in physical or emotional condition.
- Engage clients in meaningful social interactions to promote mental stimulation and emotional resilience.
- Participate in team meetings, training sessions and continuous professional development.
Kualifikasi
- Diploma or certificate in Social Work, Occupational Therapy, Nursing, or a related field.
- Prior experience working in a day care, community or residential care setting.
- Strong understanding of holistic care principles and psychosocial support techniques.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse clients.
- Compassionate, patient, and resilient mindset, with a genuine passion for helping others.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Proficient in Microsoft Office applications and basic recordâkeeping.
- Must be willing to undergo relevant background checks and meet Singapore employment requirements.
