Tanggung Jawab
- Provide skilled nursing care including medication administration, wound care, and vital sign monitoring
- Assist patients with daily living activities (ADLs) and personal hygiene
- Develop and implement individualized care plans in collaboration with healthcare teams
- Document patient progress accurately and maintain confidential medical records
- Educate patients and families on health management and preventive care
- Coordinate with physicians and other healthcare providers for continuity of care
- Monitor patient conditions and report changes to relevant stakeholders
Kualifikasi
- Valid Singapore nursing registration (SN/EN) or HCA certification
- Minimum 1 year of clinical experience in home care or community setting
- Strong knowledge of infection control protocols and safety procedures
- Excellent communication skills with patients, families, and healthcare teams
- Ability to work independently with minimal supervision
- CPR certification and valid First Aid training
- Empathy, patience, and cultural sensitivity in patient interactions
