As a Home Healthcare Doctor, you will manage a diverse caseload of patients with acute and chronic conditions. You will conduct regular home visits, assess patient health, and leverage cutting-edge telehealth solutions to provide seamless follow-up care. Tanggung Jawab
- Conduct comprehensive medical assessments and regular home visits for patients with acute and chronic illnesses.
- Formulate and execute personalized care plans in collaboration with nurses, therapists, and caregivers.
- Provide continuous medical monitoring via telehealth platforms to ensure timely interventions.
- Manage medication reconciliation and adjust treatment protocols based on patient progression.
- Educate patients and their families on disease management, preventative care, and healthy lifestyle practices.
- Collaborate with hospital-based specialists to coordinate complex transitions of care.
- Maintain accurate and timely electronic medical records (EMR) for all patient encounters.
- Participate in multidisciplinary team meetings to optimize patient outcomes and service quality.
Kualifikasi
- Possess a Medical Degree recognized by the Singapore Medical Council (SMC).
- Full or Conditional Registration with the Singapore Medical Council.
- Proven clinical experience in primary care, internal medicine, or geriatric care.
- Strong interpersonal and communication skills to effectively engage with elderly patients and their families.
- Ability to operate independently during home visits while maintaining high clinical standards.
- Proficiency in telehealth technology and standard hospital EMR systems.
- Demonstrated commitment to empathetic, patient-centered care.
- Valid driving license or ability to commute efficiently to various home locations within Singapore.
