The Manager, Future Care Planning (FCP) plays a crucial role in shaping the future care pathways for individuals with mental health conditions. Reporting to the Senior Manager, FCP, this position falls under the umbrella of Community-Based Support Services at MINDS. As a key leader in this specialized field, you will be responsible for developing, implementing, and monitoring comprehensive care planning strategies that empower clients and their families. This role requires a deep understanding of mental health care systems, excellent leadership skills, and a passion for improving the quality of life for individuals with mental health challenges. You will collaborate with multidisciplinary teams, coordinate resources, and ensure that care plans are person-centered, evidence-based, and culturally sensitive. The ideal candidate will have a strong background in healthcare management, experience in care coordination, and a commitment to promoting recovery and social inclusion. - Develop and implement comprehensive future care plans for individuals with mental health conditions
- Lead and supervise a team of care planning professionals
- Collaborate with healthcare professionals, social workers, and community partners
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments
- Ensure compliance with relevant healthcare regulations and standards
- Conduct assessments to identify client needs and develop appropriate intervention strategies
- Provide guidance and support to clients and their families throughout the care planning process
Kualifikasi
- Bachelor's degree in Social Work, Psychology, Nursing, or related field
- Minimum of 5 years of experience in healthcare or social services management
- Experience in care planning and coordination for individuals with mental health conditions
- Strong leadership and team management skills
- Excellent communication and interpersonal abilities
- Knowledge of mental health care systems and community resources
- Certification in care management or related field is preferred
