Tanggung Jawab
- Conduct thorough psychosocial assessments to understand the needs of elderly residents and their families.
- Develop and implement individualized care plans in collaboration with the multidisciplinary team.
- Provide direct counseling, emotional support, and crisis intervention as needed.
- Coordinate referrals to external agencies and community resources to enhance resident care.
- Advocate for the rights and welfare of residents within the facility and the broader community.
- Facilitate support groups and therapeutic activities to promote social engagement.
- Maintain accurate and confidential case records and documentation in compliance with organizational standards.
Kualifikasi
- Bachelor’s degree in Social Work (BSW) or a related field.
- Valid Social Work Practising Certificate (SWPC) is required.
- Minimum of 1-2 years of experience in geriatric care, social work, or a nursing home setting.
- Strong understanding of psychosocial theories and care management principles.
- Excellent communication, active listening, and interpersonal skills.
- Ability to work collaboratively in a fast-paced healthcare environment.
