Tanggung Jawab
- Conduct comprehensive psychosocial assessments for patients and families to identify social, emotional, and environmental factors affecting health outcomes
- Develop and implement individualized care plans in collaboration with physicians, nurses, and allied health professionals
- Provide counseling, crisis intervention, and emotional support to patients and their families during difficult medical situations
- Facilitate discharge planning by coordinating community resources, rehabilitation services, and follow-up care arrangements
- Advocate for patients' rights and access to healthcare services, ensuring equitable care delivery
- Document patient interactions, assessments, and interventions accurately in electronic medical records
- Participate in team meetings, case conferences, and quality improvement initiatives
- Educate patients and families about healthcare resources, support groups, and self-management strategies
Kualifikasi
- Bachelor's degree in Social Work from a recognized university; Master's degree in Social Work preferred
- Registration with the Singapore Social Work Accreditation Board or equivalent professional body
- Minimum 2-5 years of experience in medical or healthcare social work setting
- Strong understanding of medical terminology, chronic disease management, and mental health conditions
- Excellent interpersonal and communication skills with the ability to work effectively in a team environment
- Demonstrated ability to handle sensitive situations with empathy, professionalism, and cultural sensitivity
- Proficiency in Microsoft Office applications and electronic health record systems
- Knowledge of community resources, social welfare policies, and healthcare regulations in Singapore
