Tanggung Jawab
- Conduct comprehensive psychosocial assessments of patients and families
- Develop and implement individualized care plans addressing emotional and practical needs

- Collaborate with healthcare teams to ensure holistic patient care

Kualifikasi
- Bachelor's or Master's degree in Social Work
- Valid registration with Singapore Association of Social Workers (SASW)
- Minimum 2 years of medical social work experience
- Certification in crisis intervention or grief counseling preferred
- Strong assessment and case management skills
- Proficiency in electronic health record systems
- Excellent communication across diverse cultural backgrounds
