Tanggung Jawab
- Conduct comprehensive psychosocial assessments of patients to determine needs and support requirements.
- Provide counseling and emotional support to patients and their families during critical medical situations.
- Develop and implement individualized care plans in collaboration with the multidisciplinary healthcare team.
- Facilitate discharge planning and ensure a smooth transition from hospital to home or community care.
- Connect patients and families with appropriate community resources, financial aid, and support services.
- Maintain accurate and confidential documentation of all patient interactions and interventions.
- Advocate for patients' rights and ensure their voices are heard within the healthcare system.
Kualifikasi
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution.
- Valid Social Work license or certification (e.g., LSW, MSW) in Singapore.
- Previous experience in a hospital or healthcare environment is strongly preferred.
- Strong interpersonal and communication skills with the ability to work effectively with diverse populations.
- Proficiency in crisis intervention and grief counseling techniques.
- Ability to maintain strict confidentiality and handle sensitive information with discretion.
- Computer literacy and experience with electronic health records (EHR) systems.
