- Conduct comprehensive patient assessments, developing and implementing individualized palliative care plans in collaboration with the patient, family, and interdisciplinary team.
- Administer medications and treatments as prescribed, meticulously monitoring patient responses and adjusting care plans as necessary.
- Provide expert symptom management, including pain, nausea, fatigue, and other physical or psychological discomforts.
- Offer emotional, spiritual, and psychosocial support to patients and their families, fostering an environment of comfort and understanding.
- Educate patients and caregivers on disease progression, symptom management, medication administration, and other aspects of home care.
- Maintain accurate and timely patient records, ensuring compliance with all regulatory and organizational standards.
- Collaborate effectively with physicians, social workers, therapists, and other healthcare professionals to ensure coordinated and holistic patient care.
- Respond to patient and family concerns, providing guidance and resources while maintaining professional boundaries.
Kualifikasi
- Valid Registered Nurse (RN) license in Singapore.
- Minimum of 3-5 years of post-registration nursing experience, with significant experience in Intensive Care Unit (ICU) or Palliative Care settings.
- Demonstrated passion and advocacy for palliative and end-of-life care.
- Excellent clinical assessment, critical thinking, and problem-solving skills.
- Exceptional interpersonal and communication skills, with the ability to empathize and build rapport with patients and families.
- Ability to work autonomously and manage a caseload effectively in a home care setting.
- Proficiency in electronic medical records (EMR) systems and strong documentation skills.
