Tanggung Jawab
- Dispense and verify medications accurately with attention to dosage, interactions, and contraindications
- Provide comprehensive medication counseling to patients and healthcare staff on proper usage, side effects, and storage
- Manage pharmaceutical inventory including ordering, stock rotation, and waste management to ensure optimal supply
- Monitor drug therapies for efficacy and safety, recommending adjustments to clinical teams as needed
- Maintain accurate patient records and documentation in compliance with pharmacy regulations
- Collaborate with physicians and nurses to develop evidence-based treatment protocols
- Participate in hospital committees to enhance pharmacy services and medication safety initiatives
Kualifikasi
- Bachelor's degree in Pharmacy or equivalent qualification recognized by the Malaysian Pharmacy Board
- Valid practicing license from the Malaysian Pharmacy Board
- Minimum 2 years of hospital pharmacy experience preferred
- Strong knowledge of pharmaceutical regulations, drug interactions, and therapeutic guidelines
- Excellent communication skills with ability to educate diverse patient populations
- Proficiency in pharmacy management software and inventory systems
- Demonstrated commitment to patient safety and ethical pharmaceutical practices
