Tanggung Jawab
- Dispense medications accurately and provide comprehensive patient counseling on usage, side effects, and drug interactions
- Collaborate with healthcare teams to optimize medication therapy regimens and ensure patient safety
- Monitor drug utilization patterns and participate in quality improvement initiatives
- Conduct clinical rounds and provide evidence-based drug information to healthcare professionals
- Manage pharmaceutical inventory, procurement, and waste disposal in compliance with regulations
- Contribute to pharmacy department operations and process optimization
- Maintain accurate medication records and participate in adverse drug reaction monitoring
Kualifikasi
- Bachelor's degree in Pharmacy or PharmD from an accredited institution
- Valid registration with the Singapore Pharmacy Council
- Minimum 1-2 years of hospital pharmacy experience preferred
- Strong knowledge of pharmacology, therapeutics, and medication safety protocols
- Excellent communication skills with ability to counsel diverse patient populations
- Proficiency in pharmacy information systems and electronic health records
- Certification in Basic Cardiac Life Support (BCLS) preferred
