- Assess patient needs and match them with appropriate downstream community care services
- Develop and maintain referral pathways to optimize patient transitions from hospital to community care
- Collaborate with healthcare providers to ensure seamless care coordination across settings
- Contribute to strategic planning by analyzing patient flow data and service utilization patterns
- Monitor and evaluate the effectiveness of care navigation processes
- Document patient journeys and outcomes for continuous quality improvement
- Stay updated on community healthcare resources and policy changes
Kualifikasi
- Diploma or Degree in Healthcare, Nursing, Social Work, or related field
- Minimum 2 years' experience in healthcare coordination or patient navigation
- Strong understanding of Singapore's healthcare system and community services
- Excellent analytical and problem-solving abilities
- Proficient in healthcare information systems and data analysis tools
- Exceptional communication and interpersonal skills
- Certificate in Healthcare Management or Project Management preferred
