Tanggung Jawab
- Coordinate end-to-end home health operations including referrals, scheduling, and care plan implementation
- Manage physician order processing, documentation, and regulatory compliance
- Facilitate seamless communication between patients, families, and multidisciplinary healthcare teams
- Monitor patient progress and adjust care plans based on clinical assessments
- Oversee resource allocation and operational workflow optimization
- Conduct quality assurance reviews and implement process improvements
- Maintain accurate electronic health records in compliance with HIPAA standards
- Collaborate with US-based healthcare providers to ensure continuity of care
Kualifikasi
- Active RN license with minimum 3 years of clinical experience
- Certification in Case Management (CCM) strongly preferred
- Proven experience in home health or community-based care coordination
- Proficiency in EHR systems and healthcare documentation tools
- Strong analytical skills with ability to interpret complex clinical data
- Exceptional communication skills with cross-cultural competency
- Ability to work independently in a remote environment
- Knowledge of US healthcare regulations and reimbursement models
