In this pivotal role, you'll bridge the gap between clinical expertise and community care by empowering our Community Nursing Team with specialized ACP training. You'll collaborate closely with our Palliative Care Team to develop evidence-based training programs, ensuring our nurses are equipped to facilitate sensitive ACP conversations with patients and families. Tanggung Jawab
- Develop and deliver comprehensive ACP training programs for Community Nursing Team members
- Collaborate with Palliative Care Team to align training with clinical best practices
- Train nurses in facilitating ACP conversations with patients and families
- Monitor and evaluate ACP training effectiveness through quality metrics
- Create and update ACP educational materials and resources
- Coordinate interdepartmental initiatives to standardize ACP protocols
- Support implementation of ACP policies across community settings
Kualifikasi
- Bachelor's degree in Nursing, Healthcare Administration, or related field
- Minimum 3 years of clinical or healthcare management experience
- Certification or specialized training in Advance Care Planning
- Experience developing healthcare training programs
- Strong interpersonal and communication skills
- Knowledge of Singapore's healthcare system and ACP guidelines
- Ability to work collaboratively with multidisciplinary teams
