- Coordinate RISE Programme activities for post-stroke community re-integration across clinical and community settings
- Develop and implement patient-centered rehabilitation plans with multidisciplinary healthcare teams
- Monitor programme outcomes and adjust strategies based on patient progress metrics
- Collaborate with community partners to expand rehabilitation access and resources
- Lead stakeholder engagement with patients, families, and healthcare providers
- Conduct programme evaluations and prepare impact assessment reports
- Manage administrative workflows including documentation, reporting, and compliance
Kualifikasi
- Bachelor's degree in Healthcare Administration, Social Work, Rehabilitation Sciences, or related field
- Minimum 5 years' experience in healthcare coordination or community-based rehabilitation
- Strong understanding of neurological conditions and post-stroke recovery protocols
- Proven ability to manage multidisciplinary teams and inter-agency collaborations
- Experience developing and implementing patient-centered care programmes
- Excellent analytical skills with experience in outcome measurement and reporting
- Certification in Rehabilitation or Healthcare Management (preferred)
- Proficiency in healthcare information systems and data analysis tools
