Tanggung Jawab
- Conduct comprehensive psychosocial assessments to identify patient and family needs
- Develop and implement individualized care plans in collaboration with healthcare teams
- Provide counseling and emotional support to patients and their families
- Coordinate discharge planning and facilitate smooth transitions of care
- Connect patients with community resources, support services, and government assistance programs
- Advocate for patient rights and ensure access to appropriate healthcare services
- Maintain accurate case documentation and records in accordance with professional standards
- Participate in team meetings, case conferences, and interdisciplinary collaboration
Kualifikasi
- Bachelor's degree in Social Work (MSc preferred)
- Registered Social Worker with the Singapore Social Work Accreditation and Registration Board
- Minimum 3-5 years of experience in medical or healthcare social work
- Strong knowledge of psychosocial interventions and crisis management
- Excellent communication and interpersonal skills in English (additional languages advantageous)
- Ability to work effectively in a multidisciplinary team environment
- Strong organizational and case management abilities
