Allied Search Pte. Ltd. is seeking an experienced Senior Nurse Manager (Home Care) to lead our multidisciplinary home nursing team in the Central Region of Singapore. In this pivotal role, you will be responsible for the clinical leadership, operational excellence, and quality assurance of our home care services, which include gerontology, dementia, chronic disease management, and palliative care.
You will collaborate closely with physicians, allied health professionals, and community services to develop and implement individualized care plans that promote the well‑being, independence, and dignity of patients in their homes. By fostering a culture of continuous improvement and evidence‑based practice, you will drive the development of innovative home care programs that meet the evolving needs of Singapore’s aging population.
Acting as the primary point of contact for families and caregivers, you will provide education and support to ensure seamless transitions from hospital to home. In addition, you will oversee budgeting, resource allocation, and performance monitoring, ensuring that service delivery meets both regulatory standards and organizational KPIs.
You will also champion staff training, mentorship, and professional development, cultivating a high‑performing team that is passionate about delivering compassionate, evidence‑based care. Tanggung Jawab
- Lead and supervise a multidisciplinary home care nursing team to ensure high‑quality, patient‑centered care.
- Develop, implement, and monitor clinical protocols, policies, and standard operating procedures for home nursing services.
- Conduct comprehensive assessments for patients with chronic diseases, dementia, and palliative care needs.
- Coordinate with physicians, allied health professionals, and social services to create individualized care plans.
- Manage staff scheduling, performance reviews, and ongoing training to build a skilled workforce.
- Ensure compliance with Singapore Ministry of Health regulations, licensing, and accreditation standards.
- Oversee inventory, medical equipment, and supply chain for home care operations.
- Report on key performance indicators and drive continuous quality improvement initiatives.
Kualifikasi
- Registered Nurse with a valid Singapore Nursing Board (SNB) license.
- Minimum of five years of nursing experience, with at least two years in a supervisory or managerial role.
- Proven expertise in gerontology, dementia care, chronic disease management, and palliative care.
- Bachelor’s degree in Nursing or a related health discipline (Master’s degree preferred).
- Strong leadership, communication, and interpersonal skills.
- Ability to manage a geographically dispersed team in a dynamic home care setting.
- Proficient in healthcare IT systems and electronic medical records.
- Knowledge of Singapore healthcare policies and home care regulations.
