Tanggung Jawab
- Conduct comprehensive health assessments and develop tailored care plans for geriatric patients in a home environment.
- Administer prescribed medications and treatments, including injections and wound care, ensuring accuracy and safety.
- Monitor patient vital signs and clinical status closely, reporting any changes immediately to the medical team.
- Collaborate effectively with physicians, families, and caregivers to ensure holistic patient management.
- Provide education and support to patients and their families regarding disease prevention, medication management, and home safety.
- Maintain accurate and up-to-date medical records and documentation in compliance with healthcare standards.
Kualifikasi
- Registered Nurse (RN) with a valid Singapore Nursing Board license.
- Minimum of 3-5 years of relevant nursing experience, with strong exposure in Geriatrics.
- Prior experience in ICU or Oncology is strictly required.
- Strong clinical skills with the ability to work independently in a homecare setting.
- Excellent communication and interpersonal skills to interact with diverse patients and families.
- Valid Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred.
- Compassionate, patient-oriented, and able to handle high-pressure situations with professionalism.
