Join Woodlands Health as a Senior/Staff Nurse and be part of the innovative NurseFirst programme, dedicated to transforming community healthcare through early intervention and enhanced patient access to care. This is a contract position valid until 31 August 2027, offering an exciting opportunity to make a meaningful impact on patient outcomes beyond traditional hospital settings.
As a key member of our community nursing team, you will play a vital role in supporting early intervention strategies that improve access to quality healthcare for residents in the North Region. You will be responsible for delivering safe, effective clinical decision-making outside hospital walls while ensuring an exceptional patient experience throughout their care journey.
This position is ideal for experienced nurses who are passionate about preventive care, patient education, and building strong community health connections. You will work collaboratively with multidisciplinary teams to identify at-risk patients, implement care plans, and provide follow-up support that keeps patients healthy and reduces unnecessary hospital admissions.
Tanggung Jawab
- Conduct community-based health assessments and early intervention screenings for at-risk patients
- Develop and implement individualized care plans in collaboration with healthcare teams
- Provide patient education on disease prevention, medication management, and healthy lifestyle choices
- Ensure safe clinical decision-making through evidence-based nursing practices
- Coordinate with external healthcare providers and social services to facilitate seamless patient transitions
- Maintain accurate clinical documentation and patient records in compliance with regulatory standards
- Participate in community health outreach programmes and wellness campaigns
- Mentor junior nursing staff and contribute to continuous improvement initiatives
Kualifikasi
- Diploma in Nursing or Bachelor of Nursing (Nursing) from a recognized institution
- Valid nursing registration with the Singapore Nursing Board (SNB)
- Minimum 3-5 years of clinical nursing experience, preferably in community or primary care settings
- Strong critical thinking and clinical decision-making skills
- Excellent communication and interpersonal abilities with patients and healthcare teams
- Proficiency in electronic health record systems and clinical documentation
- Ability to work independently and as part of a multidisciplinary team
- Commitment to continuous professional development and evidence-based practice
