Join Singapore General Hospital as a Senior Wellbeing Coordinator and become a vital part of our patient-centered care ecosystem. You'll develop personalized wellbeing strategies, coordinate seamless care transitions, and advocate for patients' physical and emotional needs throughout their healthcare journey.

Tanggung Jawab
- Coordinate with Patient Navigators and interdisciplinary healthcare teams to deliver integrated patient care
- Develop and implement personalized wellbeing plans addressing physical, emotional, and social needs
- Facilitate seamless care transitions and discharge planning for patients
- Provide patient advocacy and support during complex healthcare encounters
- Monitor patient progress and adjust care plans based on outcomes
- Document care activities and maintain confidential patient records
- Participate in quality improvement initiatives and process optimization
Kualifikasi
- Bachelor's degree in Nursing, Social Work, Psychology, or related health field
- Minimum 3-5 years of experience in healthcare coordination or patient advocacy
- Certification in healthcare navigation or case management preferred
- Strong interpersonal skills with ability to build rapport with diverse populations
- Proficiency in electronic health records and healthcare IT systems
- Knowledge of Singapore's healthcare landscape and community resources
- Ability to work effectively in multidisciplinary teams under pressure
