Tanggung Jawab
- Conduct initial psychosocial assessments to identify patient needs and barriers to healthcare access
- Facilitate connections between patients and community resources, financial aid programs, and social services
- Provide emotional support and crisis intervention for patients and families facing acute challenges
- Maintain accurate case records and documentation while adhering to confidentiality protocols
- Collaborate with healthcare providers to develop integrated care plans addressing both medical and social needs
- Coordinate with external agencies for housing support, elder care, and disability services
- Participate in community outreach programs promoting health literacy and preventive care
Kualifikasi
- Diploma in Social Work, Psychology, Counseling, or related healthcare field
- Minimum 1 year experience in social services or healthcare support role
- Strong interpersonal skills with ability to build rapport across diverse populations
- Proficiency in case management documentation and healthcare record systems
- Certification in Mental Health First Aid or Crisis Intervention preferred
- Knowledge of Singapore's social welfare system and community resources
- Ability to work independently while maintaining collaborative team relationships
