Tanggung Jawab
- Coordinate and oversee the delivery of social work services to patients and their families within the healthcare setting.
- Conduct comprehensive social assessments and develop individualized care plans to address physical, emotional, and social needs.
- Provide professional counseling and emotional support to patients and families coping with medical challenges.
- Liaise effectively with multidisciplinary healthcare teams, medical professionals, and external community agencies.
- Advocate for patients' rights and ensure they have access to appropriate resources and financial assistance programs.
- Maintain accurate and up-to-date case documentation and records in compliance with organizational standards.
- Organize and facilitate support groups or educational sessions for patients and caregivers.
Kualifikasi
- Bachelor’s or Master’s degree in Social Work (MSW) or a related field from a recognized institution.
- Proven experience in a social work or healthcare coordination role, preferably within a public or medical setting.
- Valid Social Work license or registration with the relevant professional body in Singapore.
- Strong interpersonal and communication skills, with the ability to build rapport with diverse patient populations.
- Deep understanding of healthcare systems, medical terminology, and community support resources.
