Tanggung Jawab
- Assess patient psychosocial needs and develop comprehensive care plans
- Coordinate interdisciplinary care teams and community resource networks
- Provide crisis intervention and emotional support to patients/families
- Advocate for patient rights and access to healthcare services
- Maintain accurate documentation and case records
- Conduct home visits and follow-up care coordination
- Facilitate patient education on healthcare systems and resources
Kualifikasi
- Bachelor's degree in Social Work or related field
- Minimum 2 years healthcare social work experience
- Valid Singapore social work registration/licensure
- Strong case management and crisis intervention skills
- Knowledge of community healthcare resources and systems
- Excellent communication and interpersonal abilities
- Proficiency in electronic health record systems
