Tanggung Jawab
- Conduct comprehensive social assessments: Evaluate patient needs, family dynamics, and living situations to develop personalized care plans.
- Coordinate care services: Work closely with medical teams, doctors, and nurses to ensure seamless transitions between hospital and home care.
- Provide financial assistance: Guide patients and families through government aid programs and financial counseling options.
- Facilitate patient advocacy: Act as a strong advocate for patients, ensuring their voices are heard and their rights are protected.
- Manage discharge planning: Oversee the discharge process to ensure patients have the necessary support systems in place upon returning home.
- Maintain accurate records: Document all interactions, care plans, and referrals in the electronic health record system (EHR) with strict confidentiality.
- Support group facilitation: Organize and lead support groups for patients and caregivers to foster community and shared experiences.
Kualifikasi
- Education: Bachelor’s Degree in Social Work (BSW) or a related field is required.
- Licensure: Valid Social Work license (e.g., LSW, LCSW) or equivalent certification.
- Experience: Minimum of 2-3 years of experience in healthcare social work, case management, or patient advocacy.
- Communication: Excellent verbal and written communication skills with the ability to explain complex concepts to diverse audiences.
- Empathy: Strong emotional intelligence and the ability to handle sensitive situations with compassion and professionalism.
- Technical Skills: Proficiency in using Microsoft Office Suite and electronic health record systems.
- Problem Solving: Ability to think critically and quickly find solutions to logistical and social challenges.
