- Conduct comprehensive client assessments and develop individualized care plans
- Provide crisis intervention, counseling, and emotional support services

- Maintain accurate documentation and case records in compliance with regulations

Kualifikasi
- Bachelor's degree in Social Work or related field
- Valid registration with Singapore Association of Social Workers (SASW)
- Minimum 2 years' experience in healthcare or disability support settings
- Strong crisis intervention and counseling skills
- Proficiency in case management software and documentation systems
- Knowledge of Singapore's social services landscape and resources
- Excellent communication and interpersonal abilities
- Certification in trauma-informed care preferred
